Private healthcare in Ireland — and the clock that starts the day your policy does.

Ireland runs a two-tier system: a public service everyone can use, and private insurance that most families buy for speed and choice of consultant. The part that catches new arrivals is not the price — it is the waiting periods.

The short answer

Buy cover before you land, not after.

The average health insurance premium in Ireland was €1,902 per insured person at the end of 2025, according to the State regulator, after an average increase of around 9% during that year. For a family of four that is a meaningful annual line, though still well below what most families arriving from the US are used to paying.

The cost is the easy part. The part to act on is timing: waiting periods run from the date your policy starts. Place cover as early as the insurer permits, and a family lands already part-way through the 26-week clock instead of starting it on arrival day.

How the system works

Four things that differ from what you are used to.

  • Insurance is community rated

    Insurers must charge every adult the same price for the same plan regardless of health status or claims history, and cannot refuse you. What varies with age is not the price of the plan but a loading that applies if you first take out cover after 34.

  • The GP is the gateway

    Almost everything routes through your general practitioner, including referrals to consultants. You pay per visit unless you qualify for a medical card or GP visit card. Registering with a practice is therefore an arrival task, not an optional one.

  • Private buys speed, not different medicine

    Many consultants work in both systems and the same hospitals often host both. What you are typically paying for is a shorter wait for a non-urgent procedure and choice of consultant.

  • Cover can be a visa condition

    Some permissions — Stamp 0 for persons of independent means, among others — require private medical insurance as a condition of the permission. Where that applies, cover is not a preference.
